چکیده

purpose: to describe the clinical signs, diagnosis and treatment of toxic keratopathy in a patient who abused tetracaine drops. case report: a 32-year-old man was referred to the emergency ward of our center with persistent corneal epithelial defect associated with a ring infiltrate in the corneal stroma, without any history of ocular trauma or contact lens use. the patient was hospitalized and a thorough ocular examination was accomplished. a detailed history in our patient revealed topical anesthetic abuse keratitis. the topical anesthetic was deliberately taken from the pharmacy without a prescription. the hallmark of topical anesthetic abuse keratitis was a persistent corneal epithelial defect and a complete ring infiltration in the corneal stroma in our case. functional and anatomic results after appropriate treatment were not favorable in this case. conclusions: although topical anesthetic abuse keratitis is rare, it should be carefully explained to patients that although topical anesthetics numb the eye, more than a few drops can actually prevent healing and may lead to permanent damage. the unrestricted availability of topically applied ophthalmic anesthetics as over-the-counter medications in iran must be reevaluated.

Neurotrophic Corneal Ulcer after Topical Tetracaine Abuse: Management Guidelines

Dear Editor, Indeed, topical ocular anesthesia has been part of modern ophthalmology for more than a century. All forms of anesthetics can be toxic, particularly when abused. In Iran, the most common topical anesthetic is tetracaine. Most of the patients abuse it after UV keratitis or foreign body removal. The ocular surface and cornea are the most common sites of such toxicity. Systemic side e...

Persistent corneal epithelial defect responding to rebamipide ophthalmic solution in a patient with diabetes

OBJECTIVE Rebamipide ophthalmic suspension was developed for the treatment of dry eyes and for other corneal diseases, promoting the secretion of both mucin in tear fluid and membrane-associated mucin, increasing the number of goblet cells, and restoring the barrier function of the corneal epithelium. We report a case of a persistent corneal epithelial defect in a patient with diabetes treated ...

Amniotic membrane transplantation in the treatment of persistent epithelial defect on the corneal graft.

It has been shown that amniotic membrane transplantation (AMT) improves healing of the epithelium defects as it serves as a basement membrane for endothelial cells growth, prevents inflammatory cell infiltration and reduces apoptosis in keratocytes. Having in mind the healing properties of AM we investigated the efficacy of AMT in persistent epithelial defect (PED) on the corneal graft. 80 corn...

Autologous serum eye drops for treating persistent corneal epithelial defect after vitreoretinal surgery.

To the Editor: In the article by Oskouee et al, the authors reported the use of bandage contact lenses and topical indomethacin to treat persistent corneal epithelial defects (PED) after vitreoretinal surgery. They suggested that monocular patients, patients who require rapid visual rehabilitation, and those who have discomfort when wearing patches would benefit greatly from treatment with a ba...
